Healthy Me Healthy Communities Theory of Change
The Healthy Me Healthy Communities theory of change is based on social learning theory principles where impact crosses across different boundaries. Social learning theory moves beyond the individual perspectives that focus on the acquisition of information and instead focuses on participation in social practice. Also, the theory of change embraces the different boundaries that exist within HMHC’s practices and where these boundaries interact are potential learning experiences producing value and change.
Strategic
Collaboration between decision-makers, VCSE groups and those with lived experience for co-designing new approaches and delivery.
Level of satisfaction with motivation for attending sessions and accessing services
Knowledge of services and community offers that can resolve current issues alongside forming new friendships
Action taken by members including communication skills, accessing services and attending development opportunities
Improvements in health and wellbeing as a result of positive change to the individual's life, for example being in control of debt, accessing job support
Realised value changes behaviour and breaks a cycle of activity; and services are influenced by feedback and make changes in response
New neighbourhood resources available for sustaining action beyond the initial intervention including trained volunteers, community champions, new paid staff, additional funding and community-spaces.
